 1. Health Insurance Programs 

Blue Cross and Blue Shield of Minnesota plans Personal Blue and Options Blue both offer a choice between two Networks of doctors and hospitals: the Accord Network (includes Olmsted Medical) and the Aware Network (includes Mayo, Olmsted Medical, Rochester Methodist, St. Marys, and Hazelden). The Simply Blue plan offers only the Accord Network.

1a. Personal Blue 
Personal Blue
offers both 80/20 and 100% plans of permanent major medical insurance and 10 different deductible choicesMaternity, Labor, Delivery, Post-Delivery Care, and Maternity Complications are not covered for the first 18 months of the policy with Personal Blue.

Personal Blue 80/20 with Copay
Personal Blue 80 with Copay offers two of the lowest deductibles ($1,000 and $3,000). It features $200 per person in first-dollar coverage for preventive care, a $5 copay for formulary generic prescriptions, and a copay arrangement for office visits and retail health clinic visits. 

Personal Blue 80/20 without Copay 
Personal Blue 80 offers four deductible choices ($1,500, $2,500, $3,500, $4,500). Benefits are similar to Personal Blue 80 with Copay, including the $200 in first-dollar preventive care per person, but office visits are subject to the deductible. 

Personal Blue 100
Personal Blue 100 has four deductible choices ($4,000, $7,500, $10,000, $15,000). After the deductible is met, covered services are paid at 100 percent. Personal Blue 100 does not include $200 in preventive care.

1b. Options Blue
Options Blue is permanent, comprehensive major medical insurance that qualifies as a High Deductible Health Plan (HDHP) that is Health Savings Account (HSA)-compatible. Maternity, Labor, Delivery, Post-Delivery Care, and Maternity Complications are not covered for the first 18 months of the policy for Options Blue.

Options Blue offers five deductible options ($1,300 and $2,000 for the 80/20% plans and $2,500, $3,500, $4,500, and $5,800 for the 100% plans). Options Blue 80/20 and Options Blue 100 plans both come standard with a $300 preventive care benefit. The Health Savings Account (HSA) brochure below includes IRS-mandated annual contribution limits and a list of qualified medical expenses that can be reimbursed from your HSA account. There is no minimum requirement for your beginning deposit.

1c. Simply Blue
Simply Blue is permanent medical insurance that is targeted toward young adults and early retirees, i.e., Simply Blue does not offer family coverage, only individual per-person coverage. This plan offers some up-front benefit but has higher overall deductibles. Simply Blue offers five deductible options ($4,000 and $8,000 under the Simply Blue 80/20 plan, and $5,000, $7,500, and $10,000 under the Simply Blue 100 plan).Simply Blue does not cover Maternity, Labor, Delivery, Post-Delivery Care, and Maternity Complications. Simply Blue offers only the Accord Network of providers, which includes Olmsted Medical but does not include the Mayo Clinic.

Simply Blue 80/20%
Simply Blue 80/20 pays covered services at 80% after the deductible is met. This plan includes $300 up-front coverage for office visits and $5 copay for formulary generic drugs. Individuals also receive $250 up-front coverage for preventive care.

Simply Blue 100%
Simply Blue 100 includes $500 up-front coverage for office visits and a $5 copay for formulary generic drugs. Individuals also receive $250 up-front coverage for preventive care. After the deductible is met, covered services are paid at 100 percent.

1d. InstaCare
InstaCare is temporary insurance offered at 30-, 60-, and 90-day intervals and can be renewed for a limited time only. InstaCare can be approved quickly (usually within 48 hours of receipt of application) and offers three deductible choices for Individuals ($300, $500, and $1,000), as well as three deductible choices for family coverage ($900, $1,500, and $3,000). InstaCare does not cover Maternity, Labor, Post-Delivery Care, or Maternity Complications.
